7 is a collection
700 of mobile/laptop/desktop devices graphical user interfaces (GUIs) of provider’s
version of MyTeleDoc application illustrating a sample application of the MyTeleDoc
application according to an embodiment of the invention described herein. It
should be noted that figures in collection 700 are for illustrative purpose
only, and that additional arrangements and functionalities can be incorporated
without diverging from this concept of the present invention.

In collection 700, for example, a mobile device 702 (smartphone, tablet or laptop) or a
desktop, can provide access to the to the MyTeleDoc app via a MyTeleDoc provider’s
application icon 709. The mobile
device can be an APPLE, GALAXY, BLACKBERRY or any other manufacturers device,
with the application fully compatible in running with any operation system,

Selection of MyTeleDoc application icon 709

launch MyTeleDoc communication application can result in the display of the
main menu 710 on the MyTeleDoc
webpage 708. The main menu 710 can present the provider with
high-level menu options 712. As
shown in this example, the menu options prompt the user to either sign up
(register) 715 or sign in (existing
user) 716. A forgot password 717 feature may be added below, giving
the user an ability to change their password. provider may also be requested to
enter their email address or answer a preconfigured question and an email will
be sent to their email address with instructions to change password. Other features on this display area 704 of this screen can include company
logo 713 and company slogan 714. The application features listed
above are for illustration purposes, and that additional configurations and
functionalities can be incorporated without diverging from this embodiment of
the present invention.

A new user
clicks (selects) sign up button 715 using
the input mechanism 706, and MyTeleDoc application can result in
the display of the MyTeleDoc Application ‘provider
registration I’ interface 720.

The display area 704 of this screen
may also have company name/logo 713.

The MyTeleDoc application ‘provider registration
I’ interface 720 can allow a
provider (using the input mechanism 706)
to add their personal information in data fields 722; including but not limited to, email address, date of birth,
address, password, etc. Additional features including, but not limited to,
agreement to the MyTeleDoc terms and conditions 723 may be added. After the user has filled in all the required
field, the ‘Create Account’ button 725 may activate in display area 704, allowing the user to click and
submit their registration using the input mechanism 706. Clicking the ‘Create
Account’ button can result in display of MyTeleDoc application ‘provider registration interface II’ 730. The MyTeleDoc app ‘provider
registration interface II’ 730 may also have but not limited to, in the display
area 704 of this screen,
company/logo 713.

In MyTeleDoc ‘provider registration II’ interface 730, a new user (provider) may be prompted (using the input mechanism 706) to enter relevant personal,
professional information and accreditations 732, including but not limited to, date of birth, address, current
degrees and certifications held, work experience, etc. There may also be an
option for the new user (provider) to upload a photo 733. After completing the necessary information, the ‘Submit Button’ 735 may activate in display area 704, allowing the user to click and submit their registration using
the input mechanism 706. Clicking
the ‘Submit Button’ 735 can result in display of MyTeleDoc
‘provider registration completion’
interface 740. In the MyTeleDoc ‘provider registration completion’
interface 740, the display area 704 may include, as in this embodiment,
the company name/logo 713, and a
prompt that states “Thank you for
registering” 728. The
application features listed above are for illustration purposes, and that
additional configurations and functionalities can be incorporated without
diverging from this embodiment.

Figure
7B is collection of GUI’s that depicts the process from the
point when physician 661 selects a
provider 682 in step 678, the provider may receive a
notification 756 758 on provider’s (version of) MyTeleDoc
app 755 on provider mobile device 702. The provider’s app icon 755 is seen in the display area 704 of the provider’s device 702. The notification may be a visual
indication 756 or a sound (and/or
vibration) 758 or both on the
provider’s mobile device 702. The provider
may also receive an email notification of service request.

After the provider
682 receives a notification 756 758, he/she can click on the
provider’s app icon 755 in the
display area 704 of the provider’s
mobile device 702. After the app is
clicked, the application is activated and it can display a MyTeleDoc received ‘provider service request interface’ 760. The MyTeleDoc app ‘provider registration interface II’ 760 may also have but not limited to,
in the display area 704 of this
screen, company/logo 713.  The
prompt 762 in the display area 704 may indicate a service request
for a time and place and may or may not have the physician’s photo 661.

The display area 704 can have an accept icon 764 and/or a reject
icon 765. If the provider swipes/clicks the accept icon 764 using
the input mechanism 706, the MyTeleDoc may display the MyTeleDoc
application ‘appointment confirmed’ interface 770. The
MyTeleDoc app ‘appointment confirmed’
interface 760 may also have but not
limited to, in the display area 704 of
this screen, company/logo 713. In
addition, the MyTeleDoc ‘appointment confirmed’ interface 770 may have, but not limited to, a prompt listing the details of
the appointment 772 and another
prompt listing the details of the patient’s address 774 in the display area 704.

As the provider
accepts the appointment by using the input mechanism 706 to swipe/click the
accept icon 764, notifications are
sent to both the patient’s MyTeleDoc application 509 on the patient’s device 502
and physician’s MyTeleDoc application 655
on the physician’s device 652. The
notifications on the physician’s MyTeleDoc application 655 on physician’s
device 652 may be visual 780 or it may be sound/vibration 782 or both. The notifications on the
patient’s MyTeleDoc application 509 on patient’s device 502 may be visual 790 or it may be sound/vibration 792 or both.
